If you are new, please feel free to fill out these intake forms before our first session. These forms can also be filled out during the first session.

Cancellation and No Show Policy

When you make an appointment, time is reserved especially for you. Without significant notice, it may not be possible to fill that time with another client.  If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged for the current rate of your session.
